Online Forms
It is the policy of the Children's Center for Gastroenterology and Nutrition that medical or other information concerning any patient of the Children's Center for Gastroenterology and Nutrition will not be released to any outside person or agency without the signed consent and authorization of the patient's parent or legal guardian except in those instances where it is necessary for the continuation of care or required by law. Such information will be released only to the person or agency specified in writing by the patient's parent or legal guardian. A signed copy of the release form will be placed in the patients’ medical record.
As mandated by the Health Insurance Portability and Accountability Act (HIPAA), the Children's Center for Gastroenterology and Nutrition has a Notice of Privacy Practices that is readily available via this Internet Web site and/or on hardcopy within any one of our medical offices.

The Children's Center for Gastroenterology and Nutrition has made it easier for first time patients by offering health related forms online. Please select the appropriate form(s), print them out and bring them to your appointment already completed. By offering you this option to print the forms at home in advance of your appointment, you will be able to complete them at your own leisure.
